AMHERST, Frederick Levine M.D., 69, beloved husband of Patricia, loving father of Shira (Mark) Enstrom and Hallie (Jamie) Sklar, devoted grandfather of Tahlia and Anders Enstrom and Johanna, Theodore, and Geoffrey Sklar, passed away peacefully on September 18, 2012.  Funeral services September 20, 2012 at 1:00 pm at Wildwood Cemetery in Amherst, MA.
